Improving blood sugar in Type 2 diabetes improves cognitive scores, study says

Oct. 5 (UPI) — Controlling blood sugar levels helped people with Type 2 diabetes who were overweight improve cognitive scores, but losing weight, exercise had mixed results, a new study shows.

More than a quarter of U.S. adults 65 or older have Type 2 diabetes, which doubles the risk of cognitive impairment and dementia, including Alzheimer’s disease, according to a statement from the Pennington Biomedical Research Center.

“It’s important to properly control your blood sugar to avoid the bad brain effects of your diabetes,” said study author Owen Carmichael said in a statement.

“Don’t think you can simply let yourself get all the way to the obese range, lose some of the weight, and everything in the brain is fine,” said Carmichael, a professor and director of Biomedical Imaging at Pennington Biomedical Research Center. “The brain might have already turned a corner that it can’t turn back from.”

The study, published in the latest issue of The Journal of Clinical Endrocrinology and Metabolism, analyzed whether markers such as body weight, blood sugar control, and physical activity would be associated with improved cognition in 1,089 participants, age 45 to 76, who have Type 2 diabetes.

Researchers theorized that greater improvements in all three markers would lead to better cognitive test scores, but that turned out to be only partially true. While reducing blood sugar levels improved test scores, losing more weight and exercising more didn’t always do so.

“Every little improvement in blood sugar control was associated with a little better cognition,” Carmichael said. “Lowering your blood sugar from the diabetes range to prediabetes helped as much as dropping from prediabetes levels to the healthy range.”

Meanwhile, results from weight loss varied depending on the mental skill, according to Carmichael. More weight loss improved participants short-term memory, planning, impulse control, attention and the ability to switch tasks, but verbal learning and overall memory still declined.

“The results were worse for people who had obesity at the beginning of the study,” he added.

Similarly, Carmichael said the study showed that increasing physical activity also benefited people who were overweight more than people with obesity.

Trump improving, could be discharged Monday: doctors

US President Donald Trump has “continued to improve” as he is treated for Covid-19, his doctors said Sunday, revealing he could be discharged as early as Monday.

His medical team said his oxygen levels had dropped twice briefly in recent days and he is being treated with steroids, but they gave an upbeat assessment of the 74-year-old president’s health and outlook.

“Since we spoke last, the president has continued to improve. As with any illness, there are frequent ups and downs over the course,” said Trump’s physician Sean Conley.

Conley said the president was flown to Walter Reed military medical center on Friday after a “rapid progression” of his illness, with his oxygen levels dropping worryingly low.

He received supplementary oxygen at the White House before being admitted. 

Brian Garibaldi, another of Trump’s doctors, said the president had been “up and around” and was feeling well. 

“Our plan for today is to have him eat and drink, be up out of bed as much as possible, to be mobile,” Garibaldi said.

“And, if he continues to look and feel as well as he does today, our hope is that we can plan a discharge as early as tomorrow to the White House where he can continue his treatment course.”

White House Chief of Staff Mark Meadows had said on Saturday that Trump’s health had left the doctors “very concerned” but that he had improved and there was never a risk he would have to hand over power.

Meadows’ comments to Fox News capped a day of back-and-forth in which conflicting reports about the president’s health created widespread confusion.

Late Saturday, Conley warned that Trump was “not yet out of the woods,” but that the medical team was “cautiously optimistic.”

The president posted a video to Twitter late Saturday appearing relaxed in an open-collar shirt and blue suit jacket, telling viewers he would be “back soon” — but acknowledging that the crucial coming days would be “the real test.”

At the same time, a source familiar with the president’s health told reporters that the president’s vitals over the past 24 hours were “very concerning” and described the next 48 hours as “critical in terms of his care.” 

“We’re still not on a clear path to a full recovery,” the person said.

The statement, which was not on the record, seemed to diverge considerably from Conley’s positive assessment and quickly created confusion about Trump’s condition less than 24 hours after he was admitted to the military hospital in Bethesda, Md.

Conley did not give a date when he expects Trump to be released from the hospital, but said the president has been fever-free for over 24 hours after experiencing a fever “Thursday into Friday.” Conley declined to disclose the specific temperature of the president’s fever. 

At the first press briefing from a doctor since Trump announced his positive test early Friday morning, Conley danced around questions about whether the president had ever been on oxygen, repeatedly saying he was not currently on it. He later said that Trump had not been on it Thursday, Friday or Saturday. Conley also said that Trump has not experienced difficulty breathing. 

“More than anything, he’s felt run down,” Conley told reporters. 

Conley also raised new questions about the timeline of Trump’s diagnosis when he described Trump as “72 hours” into his diagnosis. The White House revealed Trump’s positive test in the early hours of Friday morning, which was less than 48 hours ago.

A White House official later clarified Conley’s remarks, saying that he meant to say that it is day three of the president’s diagnosis, not 72 hours. The official reiterated that Trump was diagnosed late Thursday night. 

Conley later issued a memo correcting his statement to clarify that Trump was diagnosed on Thursday evening and that he was given an experimental antibody cocktail on Friday, rather than 48 hours prior.
