Porter dentist offers free oral cancer screenings for firefighters

As a thank you to local first responders, Porter Family Dentistry is offering free oral cancer screenings to firefighters in Montgomery County for the next several weeks.

The screenings will be held on Fridays when the office is usually closed so that firefighters don’t have to wait.

In 2016, the National Institute for Occupational Safety and Health published a multi-year study of cancer rates in firefighters, and the findings showed that firefighters had a higher number of cancer diagnoses and cancer-related deaths than the general U.S. population. Among the cancers found in the sample of nearly 30,000 firefighters, those most often found were digestive, oral, respiratory, and urinary cancers.

In recent months, firefighters across the country have been traveling to areas, like California, that their help is needed. Dr. Mustafa Yamani of Porter Family Dentistry went to school in California and has fond memories of the nature and beauty of the state.

“It’s such a beautiful place, it’s really sad reading all of the stuff in the news that’s going on there,” Yamani said. “From all around the country they (the firefighters) come together and they provide this service. It’s just amazing what they’re doing and I really appreciate that. I just want to do something for them.”

The generous act of the firefighters inspired him to give back, and since oral cancer screenings are a service his office already offers he decided to give them to firefighters for free.

While this is the first year that the dental office has offered free screenings, Yamani and his wife Sabrina, who is the office manager, plan on making it an annual thing. The trials of 2020 also helped them decide to give back.

“Things seem to be going from bad to worse, to even worse, and it just doesn’t seem to be stopping for our first responders,” Sabrina said. “They’re just being hit with things one after the other.”

Sabrina started by reaching out to fire departments in the east past of Montgomery County to let them know about the opportunity and the response was immediate and positive. Already, the dental office has screenings set up with local firefighters.

Because firefighters are at a higher risk of developing cancer, many departments take an aggressive approach to screenings and check-ups. Early detection is vital. Such is the approach of the East Montgomery County Fire Department where firefighters undergo a National Fire Protection Agency physicals annually.

“It’s huge to us,” Eran Denzler, captain and PIO with the department, said of being able to get the oral screenings for free. “It’s a great show of appreciation for what we do and the risks that we take. Every day we go and put our lives on the line for the community, and for them to give back and worry about our safety is something we’re not used to but it’s much appreciated.”

The department averages around one to two structure fires a week,

Westwood BOH Offers Tips For Halloween During Pandemic

WESTWOOD, MA —Westwood’s Board of Health is offering tips to keep trick-or-treaters safe from the coronavirus while allowing them to enjoy Halloween.

In a statement to the community, the board warned residents that trick-or-treating may increase their risk of contracting COVID-19 and that Westwood is deemed a “moderate risk” for the virus, according to the state Department of Public Health. The moderate risk designation means the town has an average COVID-19 case count of between 4-8 per 100,000 people and a percent positive test rate around 1.0.

Westwood has reported 176 COVID-19 cases as of Oct. 7.

“Being that case counts fluctuate, it is difficult to look forward and predict what our case count or percent positivity rate will be at the end of October 2020. With that in mind, at this time we can only advise residents to participate in activities designated as “Lower risk” by the Center for Disease Control and Prevention (CDC) in their Fall Holiday Celebrations Guidance,” the Board of Health said.

Among the tips the board offered were wearing face masks, aside from Halloween costume masks, frequently washing your hands and trick-or-treating only within your immediate family, rather than forming groups.

For those planning on giving out candy, the board recommends placing treats at the end of your walkway or driveway instead of handing them to trick-or-treaters.

“As a final reminder, if you are going to be out at dusk on Halloween or any evening until the first frost you should use an approved insect repellent to protect your selves from mosquito bites,” the board said.

Evinacumab Offers ‘Remarkable’ Lipid Lowering in Severe HoFH

Evinacumab (Regeneron) appears to have a dramatic effect on low-density lipoprotein (LDL) cholesterol levels in patients with homozygous familial hypercholesterolemia (HoFH) with little or no LDL receptor activity, suggests a post hoc analysis of phase 3 trial data.

The results, which one expert described as a “game changer” for these patients, were presented at the European Atherosclerosis Society 2020 Virtual Congress on October 5, held online this year because of the COVID-19 pandemic.

The phase 3 ELIPSE trial showed that evinacumab, a human monoclonal antibody inhibitor of angiopoietin-like 3 (ANGPTL3), given intravenously every 4 weeks reduced LDL cholesterol levels in HoFH patients by an average of 47%.

As reported by theheart.org | Medscape Cardiology, the treatment, which was generally well tolerated, was also effective in the approximately one third of patients with minimal residual LDL receptor activity.

Now, Frederick Raal, MD, PhD, the University of the Witwatersrand, Johannesburg, South Africa, presented a post hoc analysis of 10 patients with less than 2% functional LDL receptor activity. These patients experienced an even greater reduction of 72% over placebo.

Raal said that, taken together, the results are “remarkable in these very, very difficult to treat patients,” who have “a very unmet need.” He noted that the drug is “generally well tolerated.”

“Evinacumab may provide an effective treatment option for patients with HoFH who are unable to reach target LDL cholesterol levels despite multiple lipid lowering therapies, with or without apheresis,” Raal concluded.

Anne C. Goldberg, MD, a lipidologist and professor of medicine at Washington University in St. Louis, St. Louis, Missouri, told theheart.org | Medscape Cardiology that HoFH patients with little or no LDL receptor activity, who number in the hundreds around the world, are a “very difficult group to treat because things don’t work.”

She pointed out that over 90% of patients in the trial were aged in their 30s and that the majority of them already had heart disease.

“So if you have something where you could start treating them relatively young and safely, theoretically, you could try to prevent some of these terrible cardiovascular outcomes in this group,” she said.

Goldberg highlighted that a drug such as evinacumab that can lower LDL levels in patients, often “for the first time ever, is quite a game changer.” She said that this was also seen with anti-proprotein convertase subtilisin/kexin type 9 (PCSK9) therapies among patients with HoFH.

She also noted that the safety profile of evinacumab is encouraging.

“There are always going to be people with problems with anything, but there are not a lot of side effects,” she said, underlining that this is not seen “that often.”

Summarizing, Goldberg said that evinacumab “just seems to work, I think pretty nicely,” although she added that it “would be nice if we had a better idea about the mechanism of action, because it’s still a bit unclear how it works.”

Raal explained that HoFH is rare genetic disorder primarily due to mutations in the LDL receptor that result in highly elevated

Disneyland Resort Offers Free Coronavirus Testing To Cast Members

ANAHEIM, CA —They’re the people we see walking into Downtown Disney, the ones taking temperatures, reminding you to put on your mask or socially distance amid the coronavirus slow reopening of Disneyland Resort. Now, Disneyland is showing how they are protecting their workers by offering more testing.

On Friday, the theme park shared a new offering for cast members: a free and accessible method to test for coronavirus, a spokesperson for the park tells Patch.

Starting Oct. 26, through a partnership with Quest Labs, employees can request a coronavirus test from their own homes.

The program, increasing options for testing, is all part of the Disneyland Resort’s comprehensive approach to health and safety measures, according to Dr. Pamela Hymel, Chief Medical Officer for Disney Parks, Experiences and Products.

One week ago, the Walt Disney Co. alerted workers that 28,000 of their ranks would be laid off across the country due to the ongoing coronavirus shutdown. Though Orange County and the Disneyland Resort have attempted to work with Gov. Gavin Newsom to formulate a reopening guideline, those plans were tabled by Newsom’s office. Still, employees who work behind the scenes, at Downtown Disney, in offices, and remotely from home, all who remain employed can participate in this program, should they need to be tested.

“We have taken a robust science-based approach to responsibly reopening our parks and resorts across the globe,” Hymel wrote in a prepared statement. “Our comprehensive approach emphasizes a combination of health and safety measures to help reduce risk, and with that in mind, we have finalized a plan for all working cast members of the Disneyland Resort to have access to free, easily accessible testing options.”

HCA Houston Healthcare North Cypress offers preventative gene testing for breast cancer

When Dalyss Tomayer found out three of her relatives had had breast cancer, it was enough to qualify her for the genetic testing which showed she had a high risk of breast cancer herself.

Tomayer was able to discover the gene early due to getting tested at HCA Houston Healthcare North Cypress’ High Risk Breast Clinic, where they offer genetic testing for those who qualify to see if they may have the BRCA1 or BRCA2 gene which puts people at higher risk for breast cancer.

“I had known for years that two of my dad’s sisters had breast cancer, and it was in speaking with one of my aunts that I then found out also one of my cousins had breast cancer,” Tomayer said. “So, it was at that time that I realize that I did qualified for the genetic testing and I decided to go ahead and do it.”

Janet Pollard, the coordinator for the High Risk Breast Clinic, said the breast center began as a full program under HCA Houston North Cypress’ CEO Jim Brown, who she said was passionate about having this resource.

“Most of the time people think about genetic testing after you get cancer,” Pollard said. “It can drive the type of surgeries, it can change treatment regime, but basically it’s getting it done prior. It’s screening people when they come in for their annual mammograms, talking with primary care doctors, educating them on what makes patients meet criteria.”

Criteria established by the American Cancer Society for genetic testing includes people who were diagnosed with breast cancer at a younger age, those diagnosed with breast cancer a second time, people of Ashkenazi Jewish descent, people with a family history of breast cancer, ovarian cancer, pancreatic cancer or prostate cancer, and people with a known family history of BRCA mutation.

Before the pandemic, Pollard said the center was testing around 30 people a month, men and women, with about 10 percent of people testing coming back positive.

“BRCA1 and 2 genes are huge red flags,” Pollard said. “They’re the original genes and they’ve been testing those genes for two decades.”

The process Tomayer chose to go through after being tested was to have a double mastectomy to virtually remove all chance of breast cancer, as well as a hysterectomy to eliminate her chances of ovarian cancer.

“Even though you do a double mastectomy, there’s no way to get 100 percent of every breast cell in my body, so there is still a slight chance, but it is incredibly slight,” she said.

Tomayer said she would continue to have ultrasounds for what remaining breast tissue she has left.

“I’m so very thankful they have the test,” Tomayer said. “It saved my life.”
